"Is there another form that has a place to put the APO address?"
You do not need an additional form to ship your purchase to an APO address. At checkout, select 'Edit Shipping', then 'Change Shipping Address'. When you are at the screen to enter your new address, select United States as the country, then fill in the address as follows:
Street Address: Unit # Box #
State: At the bottom of this drop down menu, you will see a military category with Armed Forces Americas, Europe, and Pacific options available. Select which is correct for you.
Postal Service (this option will appear after you select which military category you are using): APO or FPO
Postal Code: #####
